Waiver

By reading this waiver form and scheduling my appointment with Mystique 360, I acknowledge and confirm the following:

I have made my stylist aware of any treatments I have had in the past 12 months.

I have informed my stylist of allergies or skin sensitivities that I have, any medications I am taking, and any medical treatments I am currently receiving.

I understand the risks associated with the application and /or removal of chemical services, adhesives, wig application and extensions.

I grant my stylist permission to provide chemical services, use adhesive, apply wig, and install extensions to my hair and I will not hold them liable/responsible for any claims, injury, adverse health reactions or undesired results.

I understand that my stylist will do their best to achieve my desired results. However, each person’s hair and facial structure varies, so my hair may not look exactly like the picture shown or styled described.

I understand that chemicals and hair extensions may cause hair damage and maintenance may be required after this treatment.

I consent to the application, removal, and/or retouch of chemical services, adhesives, wig application and extensions by the stylist today.

I understand and acknowledge that allergic reactions, irritation, redness, itching, and tightness may occur during and/or immediately after the service.

I agree that the hairstyle is final after the service. If there are any changes after 1 hour when the service ends, the client will be charged.

I acknowledge that the hairstylist is a professional and should be treated with respect all the time.

I confirm that the hairstylist will not be responsible or liable if the result of the service is not as expected as it should be.

I confirm that I will follow the regiment and the suggested follow-ups of the hairstylist to maintain my hair.

I am allowing the hairstylist to apply necessary chemicals as part of the service in my hair treatment.

I understand that the result of this chemical may vary from one person to another.

I confirm that the hairstylist explained to me the plan of treatment, the benefits, the pros, and cons.

When applicable,I consent to the salon to take photographs of the provided service.

When applicable, I consent to the salon in terms of sharing the photograph to social media for marketing campaigns or testimonials.

Stylist warrants that all services provided hereunder will be provided in a diligent manner that meets or exceeds generally acceptable industry standards. Clients have 3 days from the date of service to reach out to stylist with concerns. Stylist has NO warranty on hair purchased anywhere other than from a stylist. The stylist will address install concerns, but the stylist holds no responsibility for any damage, matting,fading,etc..for hair purchased elsewhere.

I have read this whole document and by scheduling this appointment to have service performed, I accept the terms indicated above.
